Use
Below are
forms that we commonly use at South Columbus Elementary. Click on the name
of the form to open the form. All of the forms are interactive except the
Breakfast For All and Request for Time Off forms.

This form gives
students permission from their parents to access the Internet at school.

Computer Repair
You can submit this form
electronically. When you click on the form, click save instead of open.
Save it to your desktop, and fill it out. When you have finished, save it,
and send it to either Mary Fisher (mfisher@mcsdga.net)
or Mike Williams (cmwilliams@mcsdga.net).

Field Trip Lunch Request Form
If you need sack lunches for
an upcoming field trip, fill out this form and submit it to Ms. McBride in the
Cafeteria AT LEAST two weeks before your trip.
Hospitality Guidelines
Professional Leave Form
When you are going to be out
for Professional Leave, print off this form, fill it out, and give to either
Mrs. Klobe or Mrs. Weatherholtz to type. When they have finished typing
the form, they will return it to you. You then need to get Dr. Levins to
sign the form then send it to Professional Learning at the old Bradley Library.
This is to be done
at least two weeks BEFORE you go to the Professional Learning event.

Road to Reading
Use this form when students
in your class, and your class as a whole have made goals on the Road to Reading.
