South Columbus Elementary

Registration Day at South Columbus Elementary will be Tuesday, August 5, 2008 from 9:00 am - 2:00 pm in the Cafeteria.  Below you will find links to a few of the forms that need to be filled out to register each student.  There are a few forms that can not be put on the internet, so you will have to fill them out at registration.  If you are new to South Columbus Elementary, you will have to bring proof of address (lease agreement, mortgage agreement, water bill, power bill, gas bill, home phone bill (not cell phone bill), or cable bill).   If you are living with someone else, and do not have one of the above listed items, you must have a signed and notarized affidavit from the person you are living with stating that you do live there.   They will also have to have one of the above listed items to prove they live at that residence.  You may pick up an affidavit form from the school office, or on registration day.  If you are new to the county you will need to also bring a birth certificate, social security card, Immunization record on a Georgia Form, and an Eye, Ear, and Dental record on a Georgia form for each child.  Your child/children can not be registered and placed in a homeroom until all necessary forms are completed an turned into the school.  If you have any questions, please contact us at 706-683-8833.

You must have a copy of each form for each child you are registering.
